Job summary

Allegheny Health Network is seeking a Hospice Certified Nursing Assistant to provide compassionate, patient-centered care in patients’ homes in New Kensington, PA. The role involves working within an interdisciplinary hospice team, traveling to patient locations, and documenting care in the EMR.

You will support daily living activities, reinforce the care plan, and help patients and families navigate the hospice journey. CNA certification, PA license, and a driver’s license are required.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience; hospice experience preferred.
• A valid driver’s license and ability to drive to patient locations ~20% of time.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Assist with activities of daily living and observe/report changes to support comfort and quality of life.
• Reinforce the care plan, provide emotional support to patients and caregivers.
• Respond promptly to patient needs and document care in the EMR.
• Work independently, but not alone; travel to patient homes within the territory.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life

• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
