Job summary

AHN Healthcare@Home is seeking a Home Health CNA to deliver compassionate, patient-centered care in patients’ homes within the East Liberty/PA area. You will assist with activities of daily living, monitor patient conditions, and report changes to the interdisciplinary team, ensuring safety and independence.

The role requires an active PA CNA certification, a valid driver’s license, and at least one year of healthcare experience.

Qualifications

• Active CNA certificate in PA.
• Minimum one year healthcare experience.
• Valid driver’s license and ability to drive in home visits (about 20% of day).

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team.

• Educate and support patients and caregivers on care plans and routines.

What You’ll Do

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

CNA Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
