Job summary

Enhabit Home Health & Hospice is seeking a Nursing Leader to oversee coordinated home health services and ensure quality patient care. You will review care plans, collaborate with physicians and staff, and drive efficient scheduling to maintain continuity of care.

The role requires RN licensure, 2+ years in home health, and strong communication and technology skills. In-office presence is essential to support teamwork and operations.

Qualifications

• Graduate of an approved school of professional nursing.
• Knowledge of federal, state and local laws for home care operation.
• CPR certification and valid driver's license required.
• Basic technology skills including mobile devices.

Responsibilities

• Coordinate overall home health services per standards and procedures.
• Review patient information during start of care, recertification, resumption of care or evaluation visit.
• Facilitate the relationship between physicians, referral sources, patients, caregivers and employees.
• Schedule patients to field clinicians and coordinate communications to ensure care continuity.
