Job summary

Enhabit Home Health & Hospice is seeking a nurse leader to oversee the coordination and delivery of home health services. You will review patient information at key care milestones and act as a liaison among physicians, patients, caregivers, and field staff to ensure seamless care.

Qualifications

• Required graduate of an approved school of professional nursing.
• Must maintain a valid clinical nursing license per state or compact rules.
• Minimum two years nursing experience in the immediate previous 36 months.
• Minimum one year of home health experience.

Responsibilities

• Coordinate home health services in alignment with standards and company procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visit.
• Facilitate communication between physicians, patients, caregivers, and staff to ensure continuity of care.
