You will act as the key liaison between referral sources, patients, families, and internal teams to ensure timely initiation of services and high-quality care.

The role requires strong organization, clear communication, and experience with EMR systems to optimize workflows and improve admission conversion.

• Improve referral management workflows and daily operational efficiency.
• Provide professional and compassionate client service throughout care coordination.
• Perform other duties as assigned.
