Job summary

VitalCaring is seeking a Clinical Care Manager (RN) to lead the patient journey in home health. You will coordinate care across clinicians, manage physician orders and documentation in the EMR, and partner with Intake to support timely admissions.

As a field-based, fast-paced leader, you will conduct patient assessments, drive plan-of-care alignment, oversee case conferences, and ensure compliance with state and federal regulations while delivering exceptional outcomes for patients and families.

Qualifications

• Active RN license in state of practice.
• BSN preferred.
• Home Health experience preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communicator with the ability to collaborate across disciplines.
• Independently in a fast-paced, field-based environment.

Responsibilities

• Lead and coordinate care across the full patient journey.
• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
