Job summary

VitalCaring is seeking a Clinical Care Manager (RN) to lead the coordination of care and guide clinical decisions for home health patients. You will serve as the clinical quarterback, ensuring care is coordinated, compliant, and aligned to patient needs, with autonomy and strong processes behind you.

You will influence patient outcomes through in-home visits, collaboration with physicians, families, and care teams, while maintaining high standards of documentation and regulatory compliance.

Qualifications

• Active RN license in state of practice is required.
• BSN preferred but not required.
• Experience in home health care and care coordination is highly valued.

Responsibilities

• Lead and coordinate care across the full patient journey.
• Manage physician orders, documentation, and workflows in the EMR.
• Partner with Intake to support timely admissions and reduce delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
