VitalCaring is seeking an experienced Clinical Care Manager (RN) for home health. You will lead and coordinate the full patient journey, partner with intake, and ensure plans of care align with clinical and regulatory requirements.

Require active RN license and strong care coordination skills; BSN preferred. This on-site role offers full-time salary, comprehensive benefits, and growth opportunities within a fast-paced home health environment.

Qualifikationen

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Strong clinical judgment and ability to manage multiple priorities.

Aufgaben

• Lead and coordinate care across the full patient journey.
• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
