Qualifications

Active book of healthcare business within the territory is required.
Healthcare sales experience in home health, SNF, DME, hospice, or clinical liaison is preferred.
Strong communication, presentation, and relationship-building skills are essential.

Responsibilities

Build and maintain strong referral relationships.
Drive census growth by meeting/exceeding sales goals.
Identify resident care needs and coordinate appropriate services.
Act as a resident advocate to ensure smooth transitions to home care.
Manage the referral-to-admission process efficiently.
Collaborate with physicians, nurses, and interdisciplinary teams.
Provide education to residents, caregivers, and community partners.
Maintain consistent communication with executive leadership and referral sources.
