Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP, joining the Clinical Advancement Program. You will assess, plan, implement and evaluate care for an assigned patient group, coordinating home dialysis training and treatment per protocol, and communicating with physicians as needed.

As an ESKD care team member, you will participate in teaching, leadership, and quality improvement activities, while providing education to patients/families and collaborating with the home care

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR BLS certification.
• Must meet state practice requirements.

Responsibilities

• Provide and coordinate clinically competent care for an assigned patient group.
• Assess, plan, implement and evaluate dialysis therapy per protocol.
• Educate patients and families about home dialysis and treatment options.
• Direct and collaborate with home care partner and staff for safe dialysis delivery.
• Administer medications as prescribed and document effectiveness.
• Participate in emergency response and quality improvement activities.
• Ensure proper lab collection, processing and documentation.
• Train Home Dialysis patients and home partners on equipment use.
