Job summary

Fresenius Medical Care seeks an entry-level Home Therapies RN CAP 1 to join the Clinical Advancement Program. This role provides and coordinates care for home dialysis patients, including assessment, planning, intervention and evaluation under physician guidance.

Responsibilities include educating patients and families, coordinating with the interdisciplinary team, and ensuring adherence to policies. The position requires RN licensure and CPR/BLS certification, with readiness for on-call duties.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state licensure to practice nursing.
• CPR/BLS certification is required.
• Minimum 12 months of RN experience; 3 months in home dialysis (HD/PD) preferred.

Responsibilities

• Provide and coordinate clinically competent care for an assigned group of home dialysis patients.
• Assess, plan, implement and evaluate patient care in concert with the physician and interdisciplinary team.
• Educate patients and families on home dialysis training and treatment protocols.
• Serve as a resource, participate in staff training, and ensure compliance with policies.
