Job summary

Fresenius Medical Care is seeking a Home Therapy RN CAP 1, an entry level position into the Clinical Advancement Program. You will coordinate and provide clinically competent home dialysis care, assess patients, and educate families as part of a broader ESKD care team.

The role involves collaborating with medical staff, following protocols, and contributing to quality improvement while supporting home dialysis patients in their care plans and home environments.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR/BLS Certification required or current.

Responsibilities

• Provide and coordinate clinically competent home dialysis care per protocol.
• Assess, plan, implement, and evaluate dialysis treatment and training for patients.
• Educate patients and families on home dialysis procedures and home care safety.
• Collaborate with interdisciplinary team and communicate patient issues to physician as needed.
• Participate in on-call rotation and travel to training sites or patient homes as required.
