Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to deliver expert training and ongoing support for patients choosing home dialysis.

You will conduct home visits, assess environments, and collaborate with physicians to ensure high-quality care and safety. Responsibilities include patient and care partner education, accurate documentation in EMR, infection control, emergency preparedness, and timely communication of critical findings.

Qualifications

• RN license in the applicable state must be current and in good standing.
• 12 months of clinical nursing experience; 3 months in home dialysis modality.
• CPR certification required prior to patient care assignment.
• Ability to distinguish all primary colors (color vision).
• Complete Home Therapy Nurse Orientation and maintain annual competency.
• English language skills and written communication required.
• Basic computer skills; proficiency in USRC clinical applications within 90 days.
• Meet state practice requirements and any additional background/orientation rules.

Responsibilities

• Train patient and care partner in self-care for peritoneal dialysis or hemodialysis.
• Conduct home visits to assess the home environment and patient performance.
• Communicate results to physicians and team members as needed.
• Document nursing services in EMR with accuracy for continuity of care.
• Participate in infection control monitoring and emergency preparedness.
• Coordinate supply inventory and place orders to avoid fees.
• Support quality improvement initiatives and data collection.
• Obtain routine and non-routine labs and report critical values promptly.
• Complete monthly medication profile reviews and administer meds as ordered.
• Provide on-call coverage nights and weekends on a rotation basis.
• Maintain flexible staffing locations and hours; ensure regular attendance.
