Job summary

U.S. Renal Care is seeking an experienced Home Therapy Registered Nurse to provide patient training and ongoing support for home dialysis modalities. This role emphasizes quality care and superior service in all interactions.

You will conduct home visits, assess environments, train patients and care partners, document care in EMR, and participate in infection control and emergency preparedness in a dynamic team.

Responsibilities

• Train patient (and/or care partner) in the practice of self-care for home dialysis per guidelines.
• Conduct home visits to assess the home environment and patient performance.
• Communicate results to physician and team members for individualized care.
• Document nursing services in the EMR, including training, in-person interactions and calls.
• Participate in infection control monitoring and emergency preparedness procedures.
• Ensure adequate clinic and patient supply inventory and timely orders to avoid penalties.
• Support continuous quality improvement through data collection and trend analysis.
