Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to provide patient training and ongoing support for patients choosing a home dialysis modality. You will perform home visits, assess environments, document nursing care in the EMR, and communicate findings to the physician and care team.

The role requires an active RN license, CPR certification, English proficiency, and basic computer skills. On-call shifts nights and weekends may be included as part of the schedule.

Qualifications

• Current RN license in applicable state, in good standing.
• 12 months clinical nursing experience; 3 months in home dialysis modality (Texas exception).
• CPR certification required prior to patient care assignment.
• Ability to communicate in English, both verbally and in writing.
• Basic computer skills and proficiency in USRC clinical applications within 90 days.

Responsibilities

• Train patient/care partner in self-care peritoneal dialysis or hemodialysis.
• Conduct home visits to assess home environment and patient performance.
• Document nursing services in the EMR and ensure continuity of care.
• Communicate assessment results to physician and care team.
• Participate in infection control monitoring and reporting.
• Review Emergency Preparedness procedures with home patients.
• Ensure inventory and supply orders are placed timely.
• Support quality improvement through data tracking and analysis.
• Perform on-call nursing services nights/weekends as needed.
