Job summary

Fresenius Medical Care seeks an entry-level Registered Nurse for Home Therapies CAP 1, coordinating care for home dialysis patients and training Home Partners. You will assess, plan, intervene, and evaluate patient care under supervision, ensuring safety and adherence to protocols.

The role involves teaching patients and families, managing labs, and participating in CAP advancement activities, with travel to training sites as needed and on-call rotation.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR BLS Certification.
• Must meet the practice requirements in the state of employment.

Responsibilities

• Performs essential functions under supervision with guidance from Educator/Preceptor or RN collaborator.
• Analyzes dialysis data for assigned patients and documents in the medical record; adjusts care plan as needed.
• Assesses and educates patient/family on learning needs and care.
• Directs safe delivery of dialysis therapy with home care partners and staff.
• Administers medications as prescribed and documents justification/effectiveness.
• Initiates or assists with emergency response measures.
• Trains Home Dialysis patients and Home Partners through a formal program.
• Completes CAP requirements to advance.
