Job summary

Fresenius Medical Care in the United States is seeking a Home Therapies RN CAP 1, an entry level role into the Clinical Advancement Program. You will coordinate and deliver dialysis care, assess patient responses to training, and collaborate with physicians to ensure quality outcomes.

The position emphasizes education, leading in-home therapy training, and participating in CAP requirements to advance, with on-call and travel to patient homes as needed.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR BLS Certification.
• Must meet the practice requirements in the state of employment.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Assess and manage patients’ response to home dialysis training and treatment therapy.
• Train Home Dialysis patients and Home Partners on equipment and supplies.
• Collaborate with Interdisciplinary Team to ensure safe delivery of dialysis therapy.
