Job summary

Fresenius Medical Care is seeking an entry level Registered Nurse to join the Home Therapies CAP program. The role focuses on assessment, planning, and coordination of home dialysis care for an assigned patient group. You will train patients and families, collect dialysis data, and collaborate with the care team to optimize outcomes.

The position requires RN licensure, CPR/BLS certification, and may involve travel and on-call duties to support program needs.

Qualifications

Entry level RN with less than 2 years in nephrology nursing.
RN license and CPR/BLS certification required.
Willingness to travel and participate in on-call rotation as defined by the program.

Responsibilities

Performs duty under supervision with guidance from Educator/Preceptor.
Collects and analyzes dialysis data; adjusts treatment plan as indicated and documents in records.
Educates patients and families about home dialysis learning needs.
Directs home dialysis care with patient, partner, and home staff to ensure safe delivery of therapy.
Administers medications per protocol and documents effectiveness.
Initiates or assists with emergency response measures.
Trains patients and Home Partners on safe operation of home dialysis equipment.
Completes CAP requirements to advance.
Performs other duties as assigned by Supervisor.
