Job summary

Fresenius Medical Care is seeking an entry-level Registered Nurse for the Home Therapies CAP. The RN will coordinate care, educate patients and families, and support daily dialysis services in a home setting under supervision.

The role emphasizes adapting plans based on patient response and collaborative decision-making. Responsibilities include data collection, treatment adjustments, education delivery, medication administration, and emergency support, with CAP requirements to advance.

Qualifications

• Assess and coordinate clinical care for home dialysis patients.
• Provide education to patients and family on home dialysis protocols.
• Carry out nursing duties per physician instructions and CAP requirements.

Responsibilities

• Perform essential nursing functions under supervision and with educator guidance.
• Collect and analyze dialysis data; adjust treatment plans as needed and inform supervisors.
• Educate patients and families about learning needs for home therapies.
• Deliver safe delivery of dialysis care with home partners and staff collaboration.
• Administer medications per protocol and document effectiveness.
• Assist with emergency response measures when required.
• Support staff training and orientation for new team members.
• Ensure proper lab handling and notify if collection issues arise.
• Update nursing assessments and care plans with interdisciplinary team.
• Discuss transplant and treatment modality options with patients.
• Identify barriers in home environment and partner readiness for home dialysis.
• Train patients and partners on equipment operation and maintenance.
