Job summary

Qualifications

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation.
• Administer medications per protocol and document effectiveness.
• Educate patients and families about home dialysis training and treatment.
• Collaborate with interdisciplinary team and physicians.
• Participate in on-call rotation and travel to training sites or patient homes.
