Fresenius Medical Care in Delaware, OH is seeking a Home Therapies Registered Nurse to educate and empower patients on peritoneal and home hemodialysis, using state-of-the-art training and equipment. This on-site role includes initial home visits with another staff member and serves as a patient educator and case manager.

The RN CAP 1 entry-level program supports professional growth; applicants must be a graduate of an accredited nursing program, hold active U.S.

Formación

• RN with current state licensure and nursing degree.
• CPR/BLS certification required.
• Minimum 12 months RN experience; home dialysis experience preferred.
• Willingness to travel to patient homes and Marion, OH as needed.

Responsabilidades

• Educate patients and families on home dialysis therapies and treatments.
• Coordinate and communicate care plans with physicians and the care team.
• Administer prescribed medications and monitor treatment efficacy.
• Perform home visits and in-clinic duties as assigned; participate in on-call rotations.
• Ensure proper data collection and documentation in patient records.
