Fresenius Medical Care seeks an entry-level Home Therapies RN CAP 1 to join our Clinical Advancement Program. You will assess and coordinate care for patients on home dialysis, follow established protocols, and communicate with physicians to support treatment plans.

You will train patients and home partners, administer medications per algorithms, and contribute to team education and program goals. Benefits and professional growth opportunities are offered.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR/BLS Certification.
• Must meet the practice requirements in the state of employment.

Responsibilities

• Provide and coordinate clinically competent care for an assigned patient group.
• Assess, plan, implement and evaluate patient care within home dialysis contexts.
• Educate patients and families about home dialysis training and treatment plans.
• Administer medications per prescribed protocols and document justification and effectiveness.
• Collaborate with the interdisciplinary team and participate in quality improvement activities.
