Job summary

Fresenius Medical Care seeks a Dialysis Home Treatment / Patient Educator (RN CAP entry level) to coordinate care for an assigned group of home dialysis patients. You will assess patient responses to training, plan interventions, and communicate with physicians to optimize outcomes.

Responsibilities include educating patients and families, documenting care plans, and ensuring safe, effective dialysis delivery.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Assess patient/family learning needs and provide ongoing education.
• Coordinate and document assessment, planning, intervention and evaluation for assigned patients.
• Administer dialysis care and collaborate with the health care team to ensure safe delivery of therapy.
• Direct and support home dialysis training and education for patients and family.
• Participate in emergency response measures and quality improvement activities.
• On-call rotation and travel to training sites or patient homes as needed.
