Fresenius Medical Care is seeking an on-site Home Therapies RN CAP 2 to educate and manage dialysis patients, including initial home visits with a partner. You will assess, plan and coordinate care for an assigned patient group within the End Stage Kidney Disease team.

Ideal candidates are graduates with RN licensure, CPR/BLS, and nephrology experience, capable of leading education, quality initiatives, and working with a multidisciplinary team.

Qualificações

• Registered Nurse with nephrology nursing experience preferred.
• Experience in home dialysis education and training is a plus.
• Ability to assess, plan, implement and evaluate patient care in a collaborative setting.

Responsabilidades

• Coordinate and provide clinically competent care for an assigned patient group.
• Educate patients and families on home dialysis therapy options and procedures.
• Lead and mentor staff, acting as a resource and role model in the unit.
• Participate in CAP requirements and quality improvement activities.
