Job summary

Fresenius Medical Care is seeking a Home Therapies RN CAP 2 to coordinate and deliver clinically competent dialysis care for an assigned group of patients. This role includes patient and family education, collaboration with the interdisciplinary team, and leadership responsibilities within the Home Therapies program.

The RN CAP 2 provides safe, effective dialysis therapy, mentors staff, and participates in CAP requirements and quality initiatives across the facility, while ensuring compliance

Qualifications

• RN with nephrology nursing experience within the last 2 years
• Graduate of an accredited School of Nursing
• Current state RN licensure and CPR/BLS certification

Responsibilities

• Provide and coordinate clinically competent care for assigned patients.
• Assess and adjust dialysis treatment plans as needed.
• Train Home Dialysis patients and Home Partners on equipment and procedures.
• Provide education to patients and families about transplant and treatment options.
• Supervise and mentor junior staff and participate in quality improvement.
