Job summary

Fresenius Medical Care is seeking an RN CAP 2 for Home Therapies to coordinate and provide clinically competent care for an assigned patient group. Responsibilities include dialysis data collection, treatment plan adjustments, patient education, and collaboration with home care staff to ensure safe home dialysis delivery.

As part of the End Stage Kidney Disease health care team, the nurse participates in leadership, teaching, and quality improvement activities.

Qualifications

• Current state RN license and CPR/BLS certification.
• Entry level RN with nephrology nursing experience preferred within last 2 years.
• Fresenius policy and state/federal regs adherence.

Responsibilities

• Perform collection and analysis of dialysis data and adjust treatment plans as needed.
• Educate patients and families on care plans and home dialysis procedures.
• Coordinate with home care staff to ensure safe, effective delivery of dialysis therapies.
