Job summary

Fresenius Medical Care in the United States is seeking an experienced Home Therapies RN CAP 2 to educate, train and manage care for dialysis patients in clinic and home settings. The role involves coordinating with the multidisciplinary team to ensure safe and effective delivery of dialysis therapies and advancing CAP requirements.

The RN CAP 2 will supervise and support RNs, LVNs/LPNs and patient care technicians, participate in quality improvement activities, and engage in ongoing education

Qualifications

• Graduate of an accredited School of Nursing.
• Must hold valid RN license in the state of practice.
• CPR/BLS certification required.

Responsibilities

• Assess, plan, implement and evaluate patient care for Home Therapies patients.
• Train patients and home partners on home dialysis equipment.
• Collaborate with multidisciplinary team to coordinate care.
• Perform nursing duties in clinic and during home visits as needed.
• Lead and mentor junior staff when assigned.
