Job summary

Elara Caring is seeking a Care Team Supervisor to lead a team of caregivers delivering home-based services. You will coordinate schedules, ensure compliance with state and federal regulations, and support branch growth to expand patient access.

This role requires strong leadership, healthcare operations knowledge, and excellent time management and documentation. Bilingual abilities may be required depending on location, and some local travel is expected.

Qualifications

• High School Diploma or GED is required.
• Associate/Bachelor's degree is preferred.
• Experience working with elderly or disabled populations is preferred.
• Five years in a fast-paced office or healthcare setting is preferred.
• Knowledge of Medicare/Medicaid and home care operations is preferred.
• Bilingual fluency may be required depending on location.

Responsibilities

• Coordinate and schedule homemaking, personal care, and home health aide services, including coverage for planned/unplanned absences.
• Maintain client service records, plans of care, caregiver schedules, and required documentation.
• Review and manage contract-specific documentation and ensure approvals align with scheduled services.
• Educate clients on services, policies, and Federal/State regulations to ensure understanding and compliance.
• Support branch growth initiatives and operational planning efforts.
• Review reports and monitor systems to ensure billing, payroll, utilization, and service delivery accuracy.
• Process and manage employee timesheets and collaborate with payroll/timekeeping teams.
• Ensure ongoing compliance with all State, Federal, licensure, contract, and regulatory requirements.
• Provide guidance, coaching, and performance counseling to caregivers.
