Responsibilities

• Coordinate and schedule homemaking, personal care, and home health aide services.
• Maintain client service records, plans of care, caregiver schedules and documentation.
• Review and manage contract documents and ensure authorizations align with services.
• Educate clients on services, policies, and regulations to ensure compliance.
• Provide guidance and coaching to caregivers to promote engagement and quality care.
