Job summary

Adara Home Health, Inc. seeks an Intake Coordinator to act as a liaison to referral sources and coordinate home care services for clients. The role supports uninterrupted medical care at home and reports to the Area Manager.

The position emphasizes responding to referrals, gathering client information, and coordinating with clinicians and office staff to ensure timely assessment and start of care.

Qualifications

• Minimum of three years health care experience, including intake/home health.
• Knowledge of Medicare, private insurance and Minnesota Medical Assistance regulations.
• Excellent written and oral communication skills.
• Strong organizational skills and ability to work independently with minimal supervision.
• Proficient with computers; familiarity with Word and Excel is a plus.

Responsibilities

• Receives and coordinates all referrals and assists in arranging services for home care clients.
• Identifies appropriate services to meet client needs and informs referral sources.
• Maintains professional relationships with payers, clients and employees; communicates referral status to Area Manager.
