Qualifications

• Current NYS RN License.
• Allscripts is a MUST.
• CHHA experience is a MUST.
• Computer literacy; experience with Home Care Home Base is a MUST.
• Excellent communication skills.
• Highly organized, with strong attention to detail.

Responsibilities

• Process referrals and appropriate home care services.
• Maintain open communication with referral sources and internal Americare departments.
• Promote Americare through positive customer service.
