Job summary

Adara Home Health, Inc. seeks an Intake Coordinator to liaise with referral sources and coordinate home care services, ensuring uninterrupted medical care at home. The role reports to the Area Manager and involves evaluating referrals and coordinating with clinicians.

In-office schedule Monday–Friday 8 am–5 pm, with benefits including Paid Time Off, 401k, and health coverage. Must have 3+ years healthcare intake/home health experience and strong communication.

Qualifications

• Three years health care experience with intake and home health exposure.
• Knowledge of Medicare, private insurance and Minnesota Medical Assistance regulations.
• Excellent written and oral communication skills.
• Strong organizational skills and ability to work independently.

Responsibilities

• Receives and coordinates all referrals, including prior authorization notifications and services covered.
• Assists clients and referral sources in identifying appropriate home care services.
• Provides referral source with scope of service and coverage area.
• Maintains professional relations with payers, clients and staff.
• Enters referral information into Company software and tracks status.
• Assigns referrals to clinicians for assessment and start of care.
