Job summary

MyPath is seeking a Service Coordinator (Program Manager) in Merrill, WI to lead multiple homecare programs, supervise staff, and serve as the primary liaison between guardians, clients, and the company.

The role involves field work with documentation in the office, onboarding new hires, and ensuring services meet ISP requirements while coordinating with hospital staff for emergencies. Travel is required, and a valid Wisconsin driver's license is needed.

Qualifications

• High School Diploma or equivalent; Bachelor's preferred.
• Strong communication and organizational skills essential in a field-based role.
• Ability to travel to multiple work locations and training sessions as needed.

Responsibilities

• Interview and onboard new hires, including training on standard precautions.
• Develop and manage ISP (Individual Service Plans) for assigned clients.
• Ensure ISP delivery across multiple programs and client populations.
