Job summary

Corewell Health in Altoona, Wisconsin is seeking a full-time care management professional to plan and prioritize care for individuals and populations, coordinating with providers and community partners to improve access and outcomes. This role includes in-home, community-based, and telephonic work.

The position requires a relevant degree and Wisconsin licensure with 5 years of experience in nursing or social work, and participation in ongoing quality improvement initiatives within a managed care

Qualifications

• Bachelor's Degree or Master’s in Nursing or Social Work with 5 years of relevant experience.
• LBSW/LLBSW/LLMSW/LMSW or RN licensure in Wisconsin upon hire.
• Wisconsin licensure: LIC-Bachelor Social Worker, LIC-Master Social Worker, or RN upon hire.

Responsibilities

• Develops and prioritizes plans to meet needs and provides services.
• Follow up with families and community agencies to evaluate the effectiveness of services and plan for future needs.
• Works as part of an interdisciplinary care management team conducting in home/in community visits.
• Develops plan of care and makes recommendations to PCPs, specialists and other members of the health care team regarding care management strategies.
