Qualifications

• Wisconsin CSW licensure and 1+ year experience with Family Care populations.
• Bachelor’s degree in Human Services or related field with 1 year experience.
• Bachelor’s in any field with 3 years experience with Family Care populations.
• Ability to access members’ homes and to multitask in a fast-paced environment.

Responsibilities

• Conduct comprehensive assessments of member outcomes and needs.
• Coordinate acute and primary care services with the RN Care Manager.
• Develop and monitor member-centered care plans and authorize services.
• Coordinate care and benefits to access federal/state programs.
• Participate in on-going training and maintain DHS contract compliance.
• Create and maintain member records per DHS policy and LCI procedures.
• Coordinate home visits and care conferences to reassess long-term needs.
• Participate in the 24-hour on-call rotation.
