Job summary

BoldAge PACE is hiring a Homecare Coordinator to join our interdisciplinary team and coordinate comprehensive homecare services for participants.

You will participate in IDT care planning, monitor aide performance, and ensure care plans align with regulatory expectations and participant needs. The role includes direct home care if needed and collaboration with vendors and internal staff.

Qualifications

• Experience in mentorship, lead roles, or team coordination preferred.
• CNA, HHA, or equivalent experience required; LPN or supervision experience strongly preferred.
• 1–3 years homecare or PACE experience preferred.
• Experience providing personal care to frail elderly adults strongly preferred.

Responsibilities

• Coordinate, oversee, and monitor homecare services delivered by internal staff and contracted vendor agencies.
• Active participation in IDT care planning, assessment, and reassessment processes.
• Monitor service delivery and aide performance; identify care gaps or changes in participant status; facilitate timely care plan modifications.
• Serve as the primary point of contact for scheduling and coverage issues; provide direct care in home when necessary.
• Coordinate onboarding and orientation for internal aides and vendor; support training and education.
