Job summary

You will train and supervise direct support staff, maintain documentation for 245D requirements, and keep guardians and caseworkers informed. This role emphasizes confidentiality, compliance, and compassionate teamwork.

Qualifications

• Must be 18+ with a HS diploma or GED.
• 2+ years of experience with individuals of varying needs.
• Experience with complex medical or behavioral individuals.
• Clear background check with the MN Department of Human Services.
• Knowledge of federal and state healthcare laws and regulations.
• 245D familiarity.
• Willingness to travel to surrounding area communities.
• Vehicle, driver’s license, and valid auto insurance per the State of Minnesota.
• Clear driving record in the past 5 years.
• Fluently read, understand, speak, and write English.
• Strong communication and multitasking abilities.
• General computer skills and internet access.

Responsibilities

• Oversee management of a one or two client caseload.
• Assist individuals with scheduling of appointments and transportation needs.
• Schedule direct support staff to ensure coverage of all hours.
• Train and supervise direct support staff according to recipient needs and goals.
• Understand and implement Community Support Plans and Individual Service Plans.
• Maintain compliance with federal and state standards and laws.
• Coordinate meetings between staff, individuals, case managers, and guardians.
• Complete or create 245D-required paperwork for individuals.
• Track and chart data daily in accordance with company expectations.
• Communicate with guardians, management, and caseworkers.
• Attend and participate in team meetings as needed.
• Willingness to fill in Direct Support Professional roles as needed.
• Uphold confidentiality and privacy of individuals and staff.
