Job summary

Qualifications

• 18+ years of age
• High School Diploma, GED, or equivalent
• 2+ years of experience with individuals of varying needs
• Clear background check with state authorities
• Knowledge of federal and state healthcare laws and regulations
• Training and supervisory experience
• Willingness to travel to surrounding area communities
• Vehicle and valid auto insurance per state requirements
• Fluent reading, writing, speaking English
• Strong communication and multitasking abilities

Responsibilities

• Oversee the management of a one or two client caseload
• Assist individuals with scheduling appointments and transportation needs
• Schedule direct support staff to ensure coverage of all hours
• Train and supervise direct support staff
• Implement Community Support Plans and Individual Service Plans
• Maintain compliance with federal and state laws
• Coordinate meetings among staff, individuals, case managers, and guardians
• Gather information to support individual needs and goals
• Complete or create 245D paperwork as needed
• Track and chart daily data per company standards
• Communicate with guardians, management, and caseworkers
• Attend and participate in team meetings
• Possibly fill in Direct Support Professional roles
• Uphold confidentiality and privacy
• Perform other assigned duties
