Job summary

Learnability LLC seeks a Designated Coordinator to oversee and coordinate services for individuals under Minnesota 245D HCBS. The role ensures compliant service delivery, person-centered outcomes, and alignment with the Individual Support Plan.

You will supervise DSPs, work with guardians, case managers, and an interdisciplinary team to ensure high-quality care. The position focuses on supervision, staff training, and coordinating programs that meet individualized home supports with training

Qualifications

• Education/experience requirements in human services and supervision emphasize a strong foundation in person-centered planning.
• Demonstrated ability to supervise/direct staff and coordinate services.
• Proficiency with record-keeping, time-tracking, and compliance protocols (245D).

Responsibilities

• Oversee scheduling, EVV compliance, and 245D paperwork.
• Directly supervise and lead support staff and house supervisor.
• Provide guidance, training, and performance feedback to ensure high-quality, person-centered care.
