Job summary

Learnability seeks a Designated Coordinator to oversee and coordinate services for individuals in Minnesota's 245D HCBS program. You will supervise DSPs, collaborate with guardians, case managers, and an interdisciplinary team to ensure high-quality, person-centered support and compliance with state regulations.

You will manage scheduling, staff training, and documentation, while implementing individualized care plans and behavioral strategies.

Qualifications

• Degree requirements include a bachelor/associate/diploma with related work experience in human services.
• Supervisory or management experience in developmental disabilities or residential care is preferred.
• Strong understanding of person-centered planning and support methodologies.

Responsibilities

• Oversee scheduling, timecard approval, EVV compliance and 245D paperwork.
• Directly supervise and lead a team of support staff.
• Provide guidance, training, and performance feedback to ensure high-quality, person-centered care.
• Develop and implement individualized care plans with guardians and support teams.
• Ensure adherence to behavioral support strategies and established protocols.
• Maintain safe, respectful home environments and accurate records.
