Job summary

You will manage scheduling, contract compliance, outreach, and operational support, including intake, referrals, documentation, and coordinating with vendors, while maintaining HIPAA standards.

Qualifications

• Degree in Social Work, Healthcare Administration, or related field.
• 2–3 years in home care, case management, or healthcare coordination.
• Strong understanding of healthcare regulations and standards.
• Excellent communication, organizational, and problem-solving skills.
• Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
• Ability to manage multiple priorities in a dynamic environment.

Responsibilities

• Oversee delivery of home care services across counties and contracts.
• Ensure tailored care plans and effective communication among clients, families, and providers.
• Manage client intake, referrals, and AxisCare data entry and billing processes.
• Coordinate scheduling, service plans, and liaise with healthcare providers.
• Oversee contract compliance and documentation accuracy.
• Build relationships with community organizations and providers; represent agency at events.
• Provide operational support including phone coverage, visitor greeting, HIPAA compliance, and back-up caregiver coverage.
