A home care service provider is seeking a dedicated Client Care Coordinator responsible for managing operations and ensuring quality client services. The ideal candidate will possess an associate degree, have at least two years of related experience, and demonstrate exceptional organizational and communication skills. This role involves client intakes, supervising caregiving staff, and coordinating service evaluations to promote warmth and independence for clients in their homes.

Qualifications

• 2 years of related health care/home care experience preferred.
• Successful management experience is a plus.
• Must maintain a Valid Driver’s License and auto insurance.

Responsibilities

• Manage day-to-day office and field operations.
• Answer phone and conduct client intakes.
• Perform ongoing in-home evaluations and supervision of client services.
• Assist with caregiver scheduling and coordination.
