All American Home Care is seeking a Patient Care Coordinator to ensure clients receive consistent, reliable home care while maintaining compliance and operational accuracy. You will be the central point of coordination between clients and caregivers, helping ensure schedules are covered and documentation is accurate.

Qualifications

• Experience in home care, healthcare coordination, scheduling, or case management (preferred).
• Hands-on experience with HHAeXchange (HHAx).
• Strong understanding of EVV requirements and visit verification processes.
• Strong communication and problem-solving skills.
• Ability to stay organized in a fast-paced environment.
• Comfort using scheduling systems, spreadsheets and internal platforms.
• Bilingual English/Spanish (a plus).
• Reliable, detail-oriented, and team-oriented mindset.

Responsibilities

• Coordinate and maintain caregiver schedules to ensure full client coverage.
• Communicate daily with clients, families, caregivers, and other departments.
• Manage call-outs and last-minute coverage changes with urgency.
• Monitor open cases and assist with staffing adjustments.
• Utilize HHAeXchange (HHAx) to manage schedules and visit records.
• Monitor EVV compliance and address visit verification issues.
• Maintain accurate documentation in internal systems.
• Ensure professionalism, confidentiality, and compliance at all times.
