You will support daily scheduling, communication, and documentation in a fast-paced healthcare environment.

The role emphasizes accuracy, confidentiality, and professional interaction with families and care teams, contributing to meaningful patient care and operational efficiency.

Qualifikationen

• Experience in home care, healthcare coordination, scheduling, or case management.
• Hands-on experience with HHAeXchange (HHAx).
• Strong understanding of EVV requirements and visit verification processes.
• Strong communication and problem-solving skills.
• Ability to stay organized in a fast-paced environment.
• Bilingual English/Spanish is a plus.

Aufgaben

• Coordinate and maintain caregiver schedules to ensure full client coverage
• Communicate daily with clients, families, caregivers, and other departments
• Manage call-outs and last-minute coverage changes with urgency
• Monitor open cases and assist with staffing adjustments
• Utilize HHAeXchange (HHAx) to manage schedules and visit records
• Monitor EVV (Electronic Visit Verification) compliance
• Correct missed punches and address visit verification issues in a timely manner
• Maintain accurate documentation in internal systems
• Ensure professionalism, confidentiality, and compliance at all times
