Job summary

You will collaborate with nursing, HR, and billing to maintain service quality, manage schedules, and coordinate care plans. This full-time role follows a Monday–Friday schedule with after-hours coverage as needed.

Responsibilities

• Manage assigned home care cases and serve as a primary contact for clients, families, and caregivers.
• Recruit and screen caregivers, coordinating interviews and onboarding with HR.
• Match caregivers with clients based on care needs, qualifications, availability, and location.
• Maintain schedules and arrange replacement coverage for call-outs and open shifts.
• Monitor attendance and service delivery, addressing missed visits and scheduling concerns promptly.
• Track authorized service hours and upcoming authorization renewals.
• Review EVV records and help resolve documentation discrepancies.
• Work closely with nursing, HR, payroll, and billing to support consistent service.
• Maintain accurate records in the agency's electronic system.
• Participate in assigned after-hours coverage for urgent staffing and service needs.

Faith Home Healthcare is seeking an organized, compassionate, and proactive Home Care Case Manager / Care Coordinator to join our team. You will coordinate services for clients, recruit and schedule caregivers, and help families feel supported throughout their home care experience.

What You'll Do

• Manage assigned home care cases and serve as a primary contact for clients, families, and caregivers.
• Recruit and screen caregivers, coordinating interviews and onboarding with HR.

What You Bring

• Experience in home care coordination, caregiver staffing, scheduling, or related healthcare operations preferred.
• Caregiver recruitment experience is a strong plus.
• Excellent communication, organization, and customer service skills.
• The ability to manage changing priorities and follow through on details.
• Confidence using computers; familiarity with EMR and EVV systems preferred.
• A caring, professional approach when working with clients, families, and caregivers.
• Employment: Full-time
• Schedule: Monday-Friday, 9:00 AM-5:00 PM
• After-hours: Assigned coverage as needed
• Benefits: Available according to company policy

This position focuses on operational care coordination. Clinical assessments and care plan decisions are handled by licensed clinical staff.
