Golden Years Homemakers & Companions, Inc. in Bristol, CT, is seeking a Care Coordinator to organize admissions, coordinate caregiver schedules, and liaise with case managers to ensure continuity of care.

You will maintain client and caregiver records, manage service authorizations, and deliver updates and 60-day reports in line with agency standards. This is a full-time role offering a Christmas bonus and competitive pay based on experience.

Qualifications

• Experience in home care, healthcare coordination, or scheduling preferred.
• Strong organizational and time management skills are required.
• Excellent communication and interpersonal abilities are essential.
• Ability to multitask and work in a fast-paced environment.
• Proficiency with scheduling software and basic computer systems desirable.
• Knowledge of non-medical home care services is a plus.
