Responsibilities

  • Serve as the primary liaison between clients, families, caregivers, and healthcare professionals
  • Respond to client and family concerns promptly and professionally
  • Maintain accurate, timely documentation in the agency's care management system
  • Participate in on-call rotation for care coordination emergencies
  • Ensure compliance with state licensing regulations and agency policies

Job description

Position Summary:

The Care Manager oversees the coordination and quality of in-home care services for a caseload of clients. This role serves as the primary point of contact for clients and families, supervises assigned caregivers, and ensures each client's care plan is implemented safely and consistently. The ideal candidate combines strong clinical judgment with excellent communication and organizational skills.

Key Responsibilities:

  • Conduct initial client assessments and develop individualized care plans in coordination with families and healthcare providers Match clients with appropriate caregivers based on needs, personality, and schedule Supervise, train, and mentor caregivers assigned to your caseload
