Job summary

All American Home Care LLC in Reading, PA is seeking a skilled Care Coordination Specialist to develop and monitor home health care plans, schedule aides, and ensure coverage. You will communicate with referral sources, manage a 100+ patient caseload, and maintain strong confidentiality.

The role requires 2+ years in home care coordination, bilingual English/Spanish preferred, strong organizational and communication skills, and experience with HHA Exchange and EVV.

Qualifications

• High school graduate.
• 2+ years experience in a Home Care or Service Coordination role.
• Demonstrated capability maintaining strict confidentiality.
• Proven ability to generate leads and monitor referrals.
• Strong typing and computer skills.
• Comfortable with closing/asking for business.
• Well organized, accurate, and attentive to detail.
• Excellent communication, public relations and follow up skills.
• Experience with HHA Exchange and EVV.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide coordinated care by developing, monitoring, and evaluating home health plans.
• Ensure high-quality care delivered by home health aides.
• Schedule aides and ensure shifts are covered.
• Manage last-minute calls and coverage gaps.
• Develop on-call pool of aides in coordination with HR.
• Communicate with referral sources and Case Managers for service excellence.
• Listen to client needs and match to caregivers.
• Manage caseload of 100+ patients.
• Review care plans with patients and families.
• Address grievances and overutilization of hours.
