Caring Family Home Care Services Ll is seeking a Client Coordinator to develop and manage consumer plans of care for an assigned caseload, ensuring high-quality home care within regulatory guidelines.

You will conduct in-home assessments, document care activities in the HHA Exchange system, and liaise with Home Care Attendants to ensure accurate visit times and service delivery. Bilingual Spanish required; valid driver's license and reliable vehicle essential.

Qualificações

• At least 2 years of coordination or management experience in a health care or home care environment.
• Valid driver’s license and daily access to a reliable vehicle.
• Bi-lingual in Spanish is required.

Responsabilidades

• Maintains assigned caseload of consumers and Home Care Attendants.
• Conducts in-home assessments and wellness calls to identify care needs.
• Documents assessments and visits in the consumer record (HHA Exchange).
• Ensures accurate visit times and addresses discrepancies with attendants.
• Communicates with attendants about service hours and escalates issues as needed.
• Provides excellent consumer service and advocacy.
• Performs other department tasks as assigned.

Job Description

The Client Coordinator develops and manages consumer plans of care and provides support functions for an assigned caseload of consumers to ensure delivery of high-quality home care services in accordance with service authorizations and regulatory guidelines.

Primary Responsibilities:

• Maintains assigned caseload of consumers and Home Care Attendants
• Conducts in-home assessments and wellness calls for assigned consumer caseload to identify individual consumer care needs.
• Completes required documentation for assessments and wellness calls; enters documentation into the consumer’s record located in the HHA Exchange system and completes any related records or files.
• Contacts consumers to confirm accuracy of attendants visit time and care provided when discrepancies or incomplete entries occur.
• Communicates with home care attendants at least weekly regarding over/under utilization of service hours; documents findings and notifies manager of same.
• Addresses and resolves consumer and Home Care Attendants concerns and appropriately escalates issues to manager and/or appropriate person/department.
• Provides excellent consumer customer service and consumer advocacy.
• Perform other specific tasks related to the department as assigned.

• Excellent computer skills in a Microsoft Windows environment and ability to effectively utilize the HHA Exchange system.

Education/Experience Requirements

• At least 2 years of coordination or management experience in a health care or home care environment.
• Valid driver’s license.
• Daily access to a reliable, registered and insured vehicle.
• Bi-Lingual (Spanish)

Job Types: Full-time, Part-time

Pay: $18.00 - $20.00 per hour
