Traditional Home Health Care in Reading, PA is seeking a Care Coordinator. This full-time salaried role oversees day-to-day client service delivery, coordinates staff assignments, and ensures compliance with state regulations and company policy.

You will build relationships with clients, referral sources, payers, and the care team. Responsibilities include supervising daily EVV and payroll tasks, hiring and orienting caregivers, and maintaining service quality while expanding the client

Formación

• Previous home health care or home care experience highly desirable
• High School or GED required; Associate’s Degree preferred
• Staffing experience preferred
• Excellent verbal/written communication, time management and organizational skills
• Ability to effectively multi-task
• Effective problem solving skills
• Strong presentation and training skills
• Strong customer service skills

Responsabilidades

• Provide superior customer service and quality home care
• Build lasting relationships with clients, referral sources, payers, and community organizations
• Coordinate client services and manage scheduling issues
• Manage and grow your client caseload
• Develop strong, communicative relationships with the team
• Interview, hire, and orient reliable and compassionate caregivers
• Function as primary support for daily EVV and payroll functions
